I confirm my request for a SKI3 Assurance purchase credit.

Details: I acknowledge that after the SKI3 Assurance deadline on December 6, 2023, my Season Pass and my family members' Season Pass(es) (if included in my request), will be deactivated, and I will receive a 100% purchase credit* in the form of a Snow Guarantee Card valid through Closing Day, Spring 2025. Once a Season Pass is deactivated, it can no longer be used for lift access or Season Pass perks. You may choose to reload other lift access items onto your RFID-enabled SKI3 Card.

Your Snow Guarantee Card will be mailed to the address provided at the top of this form after the December 6, 2023 deadline. Families requesting SKI3 Assurance for multiple passes will be issued ONE Snow Guarantee Card with the total value for the requested purchase credits. Please allow up to 4 weeks for processing and mailing.

*If you have used your Season Pass (for lift access and/or pass holder perks) anytime between the date of purchase and the date of request, you will receive an 85% purchase credit.
